RADIOGRAPHY PROCEDURES

1. In the event of unintentional exposure for example as a result of mistaken identity or other procedural failure the IRMER should be notified.

2. In the event of excessive exposure ( 20* normal dose for this practice) the HSE should be informed.

3. Any incidents that are as a result of a malfunction or defect in equipment should be reported to the manufacturer so that they can fulfil their legal obligation to report to the medical devices agency.

 

4. Quality Control Programme

 

4.1  Every 3 months 10 Radiographs taken in the previous 3 mths will be assessed for:

a.                   Correct frequency

b.                  Date

c.                   Area exposed

d.                  Exposure/density/contrast

e.                   Angulation/positioning

f.                    Justification

g.                   Evaluation

 

4.2  However day to day surveillance of image quality is maintained. Any significant deterioration in quality should be investigated.

 

4.3  Any unacceptable radiographs (ie given a quality rating of 3) should be logged to include:

a.                   date

b.                   nature of deficiency

c.                   known or suspected cause of this deficiency

d.                  number of repeat radiographs if taken

 

4.4  Day to day checks will be made of important features such as

a.                   correct functioning of warning lights and audible alarms

b.                  correct operation of safety devices

c.                   satisfactory performance of the counterbalance for maintaining the position of the Xray tube

 

3.5  6 mthly records will be kept to confirm that such checks have been made
